**IMPORTANT REGISTRATION INFORMATION**
As the Program Representative you will be required to enter each of your attendees below. Please ensure that you have the correct spelling and dietary information (if applicable) for each. Names as entered will be generated into the name badges.
Include ALL attendee names at time of registration.
We understand that not all of your attendees may be approved at time of registration, but we would ask that you include names of both your known and anticipated attendees. If you do not have a name to provide please indicate in the "NAME" Fields: "To be" (1st box) "confirmed" (2nd box).
Prior to March 2, 2020, you will receive an email from the conference administrator to confirm the names of your attendees (all names must be finalized by March 2, 2020). No refunds; please review 'Terms and Conditions'.

Help the Association by paying for your conference registration by cheque or money order.
When paying by cheque/money order all conference registration fees to be paid in full by March 2, 2020. Conference registration will not be confirmed until full payment has been received. Once you have submitted your registration, you will receive confirmation by email, this confirmation will also be your invoice for payment by cheque/money order.
A $12 processing fee will apply to all orders and be applied at check-out (per order, not per delegate registration).
